Every new participant receives an individualized care plan to give them the best care. This includes a Fall Risk Assessment.
Why Our Fall Risk Assessment Is Important
According to the National Alliance on Caregiving, falls are the leading cause of fatal and nonfatal injuries for older Americans. One in four older adults fall each year, resulting in hip fractures, broken bones and head injuries among others.
This is terrible for the one who falls – and increases the burdens on caregivers.
We monitor Coleman participants to try to avoid this from happening while they are guests at Coleman Adult Day Services. When a new participant comes to us, they receive a Fall Risk Assessment. Based on this, we apply preventative practices in the way we observe your loved one and supervise their activities throughout the day. This will greatly decrease the risk of falling.
Coleman will share our assessment with caregivers and family. We can discuss ways in which you can apply the same preventative measures to your home to keep your loved one safe, healthy and mobile as possible.
